How I de-prescribe opioid drugs

Within the UK, over prescribing of opioid analgesics remains a significant issue and presents an worthy challenge to clinicians.

The long term risks of high dose opiate therapies are now fairly well established, the issue is how to safely manage this cohort of patients. There are broadly two options: stop the opiate medication altogether, the other is to lower the dose to something that is considered to be quite safe. This is usually no more than 120mg of morphine equivalent per day.
